Intramuscular vs. oral ondansetron: Which is better for vomiting control in pediatric diarrhea?

For the management of children with acute dehydrating diarrhea and vomiting, intramuscular ondansetron provides no added benefit over oral administration in improving oral rehydration outcomes.

 

According to the results of a study, intramuscular (IM) ondansetron does not offer any clinical advantage over oral ondansetron in boosting oral rehydration therapy (ORT) success among children battling acute diarrhea, vomiting, and some dehydration.

This randomized controlled, open-label trial sought to determine the efficacy and safety of a single dose of IM versus oral ondansetron in promoting ORT by controlling vomiting in pediatrics. Researchers included 60 kids (aged 3 months to 12 years) who presented with acute diarrhea lasting less than 14 days, showed signs of some dehydration, and had experienced at least 2 episodes of vomiting within the past 6 hours. Volunteers were randomly allocated to get a single 0.2 mg/kg dose of ondansetron either intramuscularly (31 children) or orally (29 children) before the initiation of ORT.

Of these, 58 children (29 per group) completed follow-up and were analyzed for all outcomes. The primary outcome—failure of ORT, defined as persistence of dehydration after 4 hours of ORT or the requirement for intravenous fluids, illustrated no statistically significant difference between the two routes (relative risk 1.3). Similarly, there were no vital differences in the need for IV fluids during ORT, the mean volume of oral rehydration salt (ORS) solution consumed after 4 hours, or the average frequency of vomiting episodes (Table 1).

Caregiver satisfaction did not markedly differ between the two groups. Importantly, no adverse events related to either route of ondansetron administration were reported. In conclusion, the findings indicate that IM administration of ondansetron offers no additional benefit over the oral route in kids affected with acute diarrhea and vomiting.
