Background

This randomized clinical trial assessed the impact of intracanal cryotherapy on postoperative pain following single-visit, non-surgical root canal treatment (NSRCT) in molar teeth diagnosed with necrosis of the pulp and inflammation of the periapical tissues.

Method

Eligible patients referred for NSRCT were enrolled. Baseline assessments encompassed preoperative radiographs, pulp vitality testing, and pain evaluation via a visual analog scale (VAS). After biomechanical preparation, 302 patients were randomly divided into 2 equal groups (n = 151). The experimental group received a final rinse with 0.9% saline solution chilled to 2.5 °C, while the control group received the same solution at room temperature.

All the procedures were carried out in one session. Pain intensity (VAS scores), duration, and analgesic use were recorded at 6, 24, and 72 hours, and at 1 week post-therapy. Any adverse events were also documented. Statistical analysis was carried out using the Mann-Whitney U test and Student’s t-test with a significance threshold of P < 0.05.

Result

Compared to the control group, the cryotherapy group experienced markedly lower levels of pain at 6, 24, and 72 hours after treatment (P < 0.05). However, no vital difference in pain was noted at the 1-week mark (P > 0.05). Throughout the trial, no adverse effects were witnessed in either group.

Conclusion

Intracanal cryotherapy appeared to be an effective, safe, and cost-efficient method for minimizing postoperative discomfort in single-visit NSRCT for molars with pulpal necrosis and symptomatic apical periodontitis.
