Key take away

Liquid PRF substantially reduces pain and improves maximum mouth opening in patients with temporomandibular disorders, especially when used adjunctively with arthrocentesis.

Background

This study evaluated the usefulness of liquid platelet-rich fibrin (liquid PRF) in temporomandibular disorders (TMD). Specifically, it investigated:

  • Whether liquid PRF provides measurable clinical benefits in patients with TMD, based on both comparative and non-comparative studies.
  • Whether, according to comparative studies, liquid PRF yields greater benefits than no therapy or other therapeutic modalities—either as a standalone therapy or as an adjunctive treatment.

Method

A comprehensive literature search was carried out across PubMed, Scopus, and Web of Science databases. Additional searches were executed on Google Scholar for gray literature and through manual screening of reference lists from suitable studies and reviews. The search covered all available data up to April 22, 2025. In total, 23 studies (19 comparative and 4 non-comparative) fulfilled the inclusion criteria and were analyzed.

Result

Evidence from the included studies indicated that liquid PRF administration offers notable improvements in pain reduction and maximum mouth opening (MMO) among patients with TMD. Most studies reported that when liquid PRF was used adjunctively with arthrocentesis, it produced substantially better clinical outcomes than arthrocentesis alone. Moreover, liquid PRF illustrated comparable or superior efficacy in alleviating pain and improving MMO compared with platelet-rich plasma and hyaluronic acid when used as a combination therapy.

Conclusion

In this narrative review, liquid PRF following arthrocentesis appeared to be a promising, minimally invasive approach for management of TMDs.
