Background

This research investigated the impact of applying 0.8% hyaluronic acid (HA) gel alongside open flap debridement (OFD) on clinical outcomes and biomarkers in the gingival crevicular fluid (GCF) of periodontitis-affected individuals.

Method

In this split-mouth study, 18 patients with Stage III Grade B or C periodontitis were enrolled. Quadrants were randomly allocated to the test group (OFD + 0.8% HA gel) or the control group (OFD alone). GCF samples and clinical assessments were examined at baseline, pre-surgery (T1), 1 month (T2), and 6 months (T3) after surgery. Utilizing enzyme-linked immunosorbent assay, the GCF levels of interleukin (IL)-6, IL-8, IL-10, IL-17A, total antioxidant status (TAS), and total oxidant status (TOS) were measured.

Result

Within both groups, treatment led to remarkable reductions in GCF IL-6, IL-8, IL-17A, and TOS, and increases in IL-10 and TAS at T1, T2, and T3 compared to baseline. The test group exhibited substantially higher IL-10 levels at T2 and T3 and lower IL-17A levels at T3 compared to controls. In this randomized clinical trial, no pivotal differences were noted between groups for IL-6, IL-8, TAS, or TOS at any time point.

Conclusion

The adjunctive use of 0.8% HA gel with OFD positively modulated GCF IL-10 and IL-17A levels in patients with severe periodontitis.
